Per Diem Physical Therapist (PT) – Home HealthMake a Difference. One Patient at a Time.

We are seeking a compassionate, skilled, and motivated Per Diem Physical Therapist to join our growing home health team. Position Summary

As a Home Health Physical Therapist, you will provide skilled physical therapy services to patients in their homes, helping them recover safely and confidently in the environment where they are most comfortable. You'll work collaboratively with physicians, nurses, therapists, and other members of the interdisciplinary team to develop personalized treatment plans that promote healing, independence, and improved quality of life.

This position includes the completion of OASIS assessments and requires accurate, timely documentation that meets Medicare and agency standards.

What You'll Do
  • Perform comprehensive physical therapy evaluations and ongoing reassessments.
  • Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge assessments in accordance with CMS guidelines.
  • Develop and implement individualized treatment plans based on each patient's goals and clinical needs.
  • Deliver evidence-based physical therapy interventions to improve strength, balance, mobility, endurance, and overall function.
  • Educate patients and caregivers on disease management, home exercise programs, safety, fall prevention, and equipment use.
  • Collaborate with physicians and the interdisciplinary care team to coordinate and optimize patient outcomes.
  • Monitor patient progress, modify treatment plans as appropriate, and communicate significant changes to the care team.
  • Complete all clinical documentation accurately and within required timeframes using the agency's electronic medical record.
  • Maintain compliance with Medicare Conditions of Participation, agency policies, and all applicable federal and state regulations.
  • Participate in case conferences and quality improvement initiatives as needed.
What We're Looking For
  • Current Florida Physical Therapist (PT) license in good standing.
  • Minimum of one (1) year of physical therapy experience; home health experience preferred.
  • OASIS documentation experience strongly preferred.
  • Working knowledge of Medicare home health regulations and documentation standards.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Excellent communication, critical thinking, organizational, and time management skills.
  • Ability to work independently while providing compassionate, patient-centered care.
