Per Diem Registered Nurse (RN)

Location: South Beach, FL

Overview:

We are a certified home health agency seeking a compassionate, skilled, and self-motivated Per Diem Registered Nurse (RN) to join our growing team. The ideal candidate will provide high-quality, patient-centered care in the home setting within our service territory.

As a per diem RN, you will be responsible for assessing, planning, implementing, and evaluating patient care in accordance with the physician’s plan of care, agency policies, and Medicare guidelines.


Responsibilities:
  • Conduct in-home skilled nursing visits including Start of Care (SOC), Recertifications, Resumptions of Care, and Routine visits.

  • Perform comprehensive physical assessments and develop individualized care plans.

  • Administer medications, treatments, and wound care as prescribed.

  • Educate patients and caregivers on disease processes, self-care techniques, medications, and health maintenance.

  • Collaborate with interdisciplinary team members including therapists, aides, and physicians.

  • Document all patient care accurately and timely using our EMR system in accordance with Medicare and agency requirements.

  • Participate in case conferences, staff meetings, and continuing education as required.

  • Ensure compliance with all federal, state, and agency regulations and policies.

Schedule & Compensation:
  • Per Diem / PRN position – Flexible scheduling based on your availability and agency needs.

  • Competitive per-visit rates for each type of skilled nursing service (SOC, ROC, Recert, Routine, etc.).

  • Current RN license in the State of Florida.
  • Minimum of one (1) year of clinical nursing experience; home health experience strongly preferred.

  • Knowledge of Medicare guidelines and OASIS documentation preferred.

  • Reliable transportation, valid driver’s license, and current auto insurance.

  • Strong organizational and time-management skills.

  • Ability to work independently with minimal supervision.

  • EMR proficiency (experience with home health software such as Kinnser, WellSky, or similar is a plus).
