Overview:We are a certified home health agency seeking a compassionate, skilled, and self-motivated Per Diem Registered Nurse (RN) to join our growing team. The ideal candidate will provide high-quality, patient-centered care in the home setting within our service territory.
As a per diem RN, you will be responsible for assessing, planning, implementing, and evaluating patient care in accordance with the physician’s plan of care, agency policies, and Medicare guidelines.
Responsibilities:
Conduct in-home skilled nursing visits including Start of Care (SOC), Recertifications, Resumptions of Care, and Routine visits.
Perform comprehensive physical assessments and develop individualized care plans.
Administer medications, treatments, and wound care as prescribed.
Educate patients and caregivers on disease processes, self-care techniques, medications, and health maintenance.
Collaborate with interdisciplinary team members including therapists, aides, and physicians.
Document all patient care accurately and timely using our EMR system in accordance with Medicare and agency requirements.
Participate in case conferences, staff meetings, and continuing education as required.
Ensure compliance with all federal, state, and agency regulations and policies.

- Current RN license in the State of Florida.
Minimum of one (1) year of clinical nursing experience; home health experience strongly preferred.
Knowledge of Medicare guidelines and OASIS documentation preferred.
Reliable transportation, valid driver’s license, and current auto insurance.
Strong organizational and time-management skills.
Ability to work independently with minimal supervision.
EMR proficiency (experience with home health software such as Kinnser, WellSky, or similar is a plus).
