Use your excellent communication and advocacy skills to partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care which includes health promotion, disease prevention, and disease management activities.

Position: Registered Nurse RN - Care Coordination
Shift: Per Diem, Varied Shifts; mainly weekends, some weekdays to cover.  16 hours/month +  one weekend per month + 1 holiday per year 
Experience: 
1. Three years of recent clinical experience required. 
2. Leadership experience preferred. 
Education: 
1. Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
2. Master's Degree preferred. 
Licensures/Certifications: 
Active PA RN license required
