Assisteo Home Health is looking for a Full-time or Per Diem Field RN Case Manager in the Phoenix Metro and surrounding areas!
DESCRIPTION SUMMARY
The home health Registered Nurse plans, organizes, and directs home care services in the patientโ€™s home environment, based on providerโ€™s orders. These services can be in an independent home, assisted living facility, or group home. They are experienced in nursing, with an emphasis on community health education/experience. The home health Registered Nurse builds from the resources of the community to plan and direct skilled nursing services to meet the needs of individuals and families. The visits are made on a per visit basis.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
-Provides direct patient care such as wound care, medication management, education related to diagnosis, PICC line infusions, etc.
-Completes an initial assessment of patient (OASIS based on data obtained if needed) and performs a medication reconciliation.
-Acts as Case Manager and assumes responsibility to coordinate patient care.
-Develops a care plan, which establishes goals based on clinical diagnosis and incorporates therapeutic, preventive, and rehabilitative care.
-Communicates with physician/nurse practitioner regarding the pati
