JOB TITLE: Middle School Nurse 

JOB GOAL: Provide program, case manager/direct care provider, collaborator/advocate, and  educator on system-wide health services issues. 

REPORTS TO: Principal, Director of Student Services 

QUALIFICATIONS: Qualified to practice as a Registered Nurse in the State of Illinois and hold an unrestricted license. Certified school nurse certification preferred, not required.

Read, understand and follow verbal and written directions. 

Utilize applicable technology to support performance responsibilities. 

DUTIES AND RESPONSIBILITIES: 

General 

  1. Communicate and collaborate effectively with others.
  2. Ensure confidentiality of sensitive information.
  3. Maintain a record of dependability as evidenced by consistent attendance, punctuality and attention to work.
  4. Understand and adapt to a variety of situations.
  5. Maintain and exhibit general and personal safety practices.
  6. Maintain stakeholder satisfaction.
  7. Work independently and exercise good judgment.
  8. Assume personal responsibility for professional performance and growth.
  9. Utilize effective problem solving processes.
  10. Exhibit professionalism in appearance and demeanor. 

Health Records and Requirements

  1. Maintain accurate medical, vision and dental records to assure compliance with state mandates and the Illinois School Student Records Act including immunizations, physical examinations, and medical conditions.
  2. Accurately document services rendered, interpret and explain records, reports, activities, health care plans, accommodations and medical interventions.
  3. Provide screening and follow up for vision, hearing, growth and development, and other physical deficits, maintaining hearing and vision certification in order to conduct screenings.
  4. Participate in Child Find programs; establish and manage state mandated health-screening programs.
  5. Support the development of health policies/procedures to comply with mandates and health practices.
  6. Implement blood borne pathogens and other interventions as required by OSHA.
  7. Follow school health law, as well as state and district policies and guidelines. 

Immediate Health Care for Students, Staff, Parents

  1. Provide timely direct nursing services, first aid, illness, and emergency care of students and staff members
  2. Administer medications according to physician's orders.
  3. Provide health information and counseling for students, parents, and staff.
  4. Develop a medication protocol to safely store, administer, document, and monitor the effectiveness of  medication given at school.
  5. Serve as a member of the multidisciplinary team. Write the health component of the IEP, attend and contribute to student support team meetings.
  6. Recommend modifications of the school program for students who require accommodations.
  7. Develop/maintain health care plans for students who need special nursing interventions.
  8. Authorizes exclusion and readmission of students with infectious and contagious diseases.

Overall Health and Safety Program Resource

  1. Participate with administration and other school staff in developing and administering a health, counseling and crisis intervention program.
  2. Collect and analyze epidemiological and other health information; make recommendations based on  data. Confer with students, parents, staff, physicians, clinics, and other agencies on school health issues.
  3. Maintain user-friendly, organized health services conducive to confidential communication and services.
  4. Identify the healthcare needs of students and the availability of community health care services/resources.
  5. Inform staff of any health concerns of students including allergies, asthma, emergency plans, seizures, diabetes, and any physical impairment.
  6. Maintain CPR, AED, and First Aid instructor's license and teach staff and 8th graders CPR prior to graduation.

Attendance

  1. Monitor absences and submit daily attendance into the database.
  2. Follow up with families and other agencies on unreported absences within two hours.
  3. Maintain attendance records for students entering or leaving the building during the school day for any reason.
  4. Inform principal/assistant principal of any excessive tardiness or absences.

Physical Requirements

While performing the duties of this job, the employee is occasionally required to sit, walk, climb, squat, or kneel.  The employee regularly is required to walk and stand.  The employee frequently is required to reach with hands and arms, and will repeat the same hand, arm, wrist, or finger motion many times.  The employee must occasionally lift and/or move up to 20 pounds.  Specific vision abilities required by this job include close vision, depth perception, and the ability to focus.

Miscellaneous

1. Prepares medical concerns list for all field trips and distributes medical necessities to teachers that may be needed during field trips including outdoor education and 8th grade trip.
2. Attend 1-2 overnight field trips per year to support student needs.
