Care Finders Total Care LLC, a Modivcare Home care company in NJ is looking for a Per Diem Clinical Supervisor. The RN field nursing supervisor is responsible to ensure quality and safe delivery of home health Care services such as supervising Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and Psychosocial needs.

  • Visiting Client Territory: Middlesex County areas
  • Schedule: 2-3 days per week. Visits will be coordinated to ensure the patient's aide is present.
  • Office Address: 325 Madison Ave. Perth Amboy, NJ 08861
  • Pay per visit: $45 per Regular Visit, 2 Month Reassessment, and Supervision Visits; $55 per Reassessment Visit; and $60 per Initial RN Assessment.
  • Mileage Reimbursement: Provided at the current federal rate

You will...

  • Performs on-site supervisory and teaching visits to evaluate caregivers
  • Provides clinical orientation to caregivers on assigned home care cases. 
  • Participates in the training of new nurses and in-services for field staff.
  • Performs initial nursing assessments
  • Counsels patient/caregiver in meeting nursing and related needs.
  • Assigns and coordinates services provided.
  • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. 
  • Prepares clinical progress notes ensuring any changes in the patient's condition is reported 
  • Administers medication and treatments as prescribed and initiates appropriate preventative and rehabilitative nursing procedures.
  • Monitors clinical records for complete and accurate documentation.

  • Registered Nurse (RN)
  • 1 year of RN experience
  • CPR certification
  • 1 year of home care experience preferred
  • Bilingual (Spanish) Preferred
