Job description

  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine

  • Full-time employed position in a geriatric outpatient practice

  • 100 percent outpatient care only

  • Monday through Friday, 8 a.m. to 5 p.m. schedule

  • Holidays and weekends free

  • Rotating and flexible on call schedule

  • Average daily volume of 15 patients

  • Focus on continuity of care, health maintenance, disease prevention, and chronic disease management for a primarily Medicare aged population

  • Team based multidisciplinary model serving senior patients with multiple comorbidities

  • eClinicalWorks EMR

  • Salary around $250,000

  • Annual performance bonus based on quality of care

  • Full benefits offered

  • Negotiable relocation and sign on bonuses

  • Malpractice insurance coverage provided

  • Active DEA registration and BLS CPR certification required
