We are seeking a Registered Nurse to join our team! You will be responsible for the assessment, diagnosis, and care of assigned patients.

Position Summary

Second Family Nursing Services is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated care team. In this role, you will be responsible for the comprehensive assessment, treatment, and ongoing clinical management of children and adults with complex medical needs. Many of the individuals in our home care are technologically dependent, and you will play a vital role in ensuring their safety, dignity, and quality of life.

As an RN with Second Family, you'll provide direct nursing care in a residential setting designed to feel like home. You'll collaborate closely with physicians, families, and support staff, while also mentoring and training caregivers on specialized care procedures. Rate

Our rate is based on Skill Competence Review Check; This Skill Competence check will determine your Level of practice which determines the PDN case/assignment.


Required Qualifications

  • Provide compassionate, high-quality nursing care to technologically dependent children and/or adults in a home setting
  • Accurately monitor, assess, and document changes in client condition, symptoms, or behavior
  • Communicate effectively with physicians, specialists, and interdisciplinary team members to ensure continuity of care
  • Educate fellow staff members on health maintenance, disease prevention, and condition-specific protocols
  • Actively participate in care planning meetings, case conferences, and team sessions
  • Maintain accurate and timely documentation using electronic medical records (EMR) systems
  • Demonstrated experience in nursing, long-term care, or relevant medical settings
  • Must hold current and valid certification: LPN and/or RN
  • Adhere to agency protocols, state regulations, and ethical standards in all care activities


Preferred Qualifications

  • Proficiency with EMR platforms, mobile health applications, and digital charting tools (e.g., tablets, software systems)
  • Proven ability to build trust and rapport with clients, families, and teams
  • Strong clinical judgment, problem-solving abilities, and critical thinking in dynamic care environments
  • Comfortable working independently and collaboratively in a fast-paced, high-responsibility setting
  • Competent typing and documentation skills with attention to accuracy and detail
