Patient Liaison
Revitalize BodyMD – Freehold, NJ

Full-Time

Are you passionate about helping people transform their health? Do you thrive on building relationships and delivering exceptional customer service? Revitalize BodyMD is seeking a compassionate, organized, and proactive Patient Success Manager to guide patients through their wellness journey.
As the primary point of contact for our patients, you will ensure a seamless, concierge-level experience—from the first phone call through ongoing care. You'll work closely with our physician and clinical team to coordinate treatment plans, support patient education, and help patients stay engaged with their health goals.
Responsibilities
  • Build strong relationships with patients and provide exceptional service.
  • Guide new patients through the consultation and onboarding process.
  • Coordinate follow-up appointments, lab work, and medication refills.
  • Track hormone therapy, weight loss, IV therapy, peptide, and regenerative medicine patients to ensure they stay on schedule.
  • Manage patient communication via phone, email, text, and patient portal.
  • Maintain accurate documentation in Cerbo EMR.
  • Support patient retention, memberships, referrals, and review requests.
  • Collaborate with the physician and team to deliver an outstanding patient experience.
Qualifications
  • 2+ years of experience in healthcare, medical office, concierge medicine, aesthetics, or wellness preferred.
  • Outstanding communication and interpersonal skills.
  • Highly organized with excellent attention to detail.
  • Strong computer skills; EMR experience (Cerbo preferred).
  • Positive attitude, problem-solving mindset, and ability to multitask.
  • Passion for wellness, hormone optimization, and helping patients achieve their health goals.
