Job description

Patient Flow Coordinator

The Patient Flow Coordinator facilitates patient flow by ensuring appropriate utilization of resources with regards to the patient admission process and taking into account a "global view" of bed utilization, staffing resources and patient acuity. In addition the Patient Flow Coordinator acts as a resource for all legal, clinical, and ethical issues relative to hospital operations. His/her will act as an expert clinician and resource to staff by responding to any and all calls for support or assistance. The incumbent provides transformational leadership that promotes patient centered clinical nursing practice and service excellence standards that are evidence based to achieve quality patient outcomes. The Patient Flow Coordinator may attend Multidisciplinary Team Rounds as needed and conducts afternoon unit rounds to ensure follow-up for discharge readiness.

Under the direction of the Director of Patient Flow & Capacity and in accordance with nursing practice standards established by the Nursing Organization, rules and regulations of NH State Board of Nursing, DNV, and Catholic Medical Center system's management criteria, the incumbent performs the following:

Consistently and proficiently demonstrates communication and collaboration, demonstrates a commitment to the quality care processes, demonstrates the ability to meet the educational needs of staff, demonstrates a commitment to collaboration, promotes professional development and the image of nursing and consistently demonstrates professionalism.

Work Schedule:

  • (7AM - 7PM)

Qualifications:

Education:

  • Graduate from an approved school of nursing.
  • BSN required, MSN preferred.

Experience:

  • 10-12 years nursing experience.
  • 3-5 years nursing supervisory experience.

Licensure/Certification:

  • Current registration in the State of NH as a Registered Nurse.
  • Certified in specialty area strongly preferred.
  • BLS (Basic Life Support) certification required. Must maintain BLS throughout employment.
