Job description

We are GROWING and have new positions available to expand our Primary Care Action Teams. TVFHT is one of Ontarios largest and most dynamic family health teams, serving patients across over 21 sites in four counties. Our interdisciplinary teams including physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians, and other allied professionals work together to provide comprehensive, patient-centered care. We are committed to system integration, health equity, and innovation in primary care delivery.

This position would initially start at 1051 Wonderland Rd. South in London, Ontario.

SKILLS & QUALIFICATIONS REQUIRED:

  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)

  • Bachelor of Science in Nursing (BScN) preferred

  • Expanded scope of RN prescribing an asset

  • An interest and experience in both individual and group patient education programming

  • Strong clinical assessment skills and experience practicing to full scope and implementing medical directives

  • Knowledge and experience working collaboratively with community resources

  • Understanding of the Chronic Disease Management and Patient Self-Management Models

  • Proficiency with EMRs and virtual care technologies

  • Excellent interpersonal, communication and documentation skills and motivation to work as an effective team member

  • Valid driver's license, regular access to a vehicle and appropriate insurance

  • Ability to work on site within a fast-paced, team-based model

Hourly Rate: $34.8138 - $42.4559
