For over 25 years, Pathways Hospice & Palliative Care has been enhancing the lives of patients and their families by combining the most advanced medical care with a focus on compassion and comfort. We are seeking Full-Time Hospice RN Case Managers.


As a RN Case Manager, you will:

  • Visit with patients in their homes or nursing facility setting, providing assessment, teaching and skilled care
  • Work with and receive support of other professionals to problem solve and update the patient's plan of care
