LPN, Client Relationship Specialist

Schedule: Friday, Saturday, Sunday, Monday โ€” 8:30amโ€“5:00pm

Every member of our team plays a vital part in making that happen.

As the Client Relationship Specialist, you become the driving force behind those transitions. Youโ€™ll build strong relationships with community partners, ensure referrals are processed quickly and accurately, and position HCR as the provider of choice for patients needing home care. Working Friday through Monday, 8:30amโ€“5pm, youโ€™ll support high-volume referral periods where your expertise has a direct and meaningful impact.

What Youโ€™ll Do

โ€ข You will build trusted relationships with physicians, hospitals, and rehab partnersโ€”helping them confidently choose HCR for their patientsโ€™ home care needs.

โ€ข You coordinate and process incoming referrals quickly, accurately, and professionally across multiple service lines throughout New York State.

โ€ข You will communicate proactively with referral sources and internal teams, ensuring a seamless, customer-focused experience from initial call to service initiation.

โ€ข You secure the resources patients need by arranging durable medical equipment, oxygen, and other critical services with outside vendors.

โ€ข You will verify insurance coverage and collaborate with Insurance Specialists to streamline authorizations and reduce delays.

โ€ข You document clearly, follow all regulatory requirements, and contribute solutions and insights during team and agency meetings.

โ€ข You maintain expert knowledge of HCR programs and services, positioning yourself as a reliable, go-to resource for referral partners.

โ€ข You perform other related duties as assigned.
