Help patients regain mobility, confidence, and independence while serving two close-knit communities in southeastern Minnesota. Blue Stone Therapy is seeking a Physical Therapist to join our rehabilitation team serving Spring Valley Care Center and Grand Meadow Senior Living.

What You’ll Do

  • Evaluate patients and develop individualized plans of care
  • Deliver evidence-based physical therapy focused on improving mobility, balance, strength, safety, and functional independence
  • Provide rehabilitation services at Spring Valley Care Center and Grand Meadow Senior Living
  • Collaborate with physicians, nursing staff, families, and interdisciplinary team members to achieve the best patient outcomes
  • Supervise Physical Therapy Assistants in accordance with state licensure requirements
  • Complete timely, accurate documentation supporting quality patient care and regulatory compliance
Requirements:

Minimum Qualifications

  • Graduate of an accredited Physical Therapy program
