Occupational Therapist Assistant (COTA) - in Lake City, MN

Blue Stone Therapy is seeking a FULL OR PART TIME, Occupational Therapist (OT) to provide rehabilitation services to adult and geriatric patients at Lake City Care Center.


This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Occupational Therapist is responsible for assessments, care plan development, and implementation of treatment programs designed to improve activities of daily living (ADLs), cognitive function, upper extremity strength, range of motion, positioning, and overall functional independence.


What You’ll Do

  • Perform occupational therapy evaluations and develop individualized plans of care
  • Provide skilled occupational therapy treatment focused on functional and measurable outcomes
  • Address ADLs, functional mobility, cognition, strength, positioning, and discharge planning
  • Supervise Occupational Therapy Assistants in accordance with regulatory and company standards
  • Maintain appropriate patient schedules per established guidelines
  • Complete timely, accurate documentation to support quality care and reimbursement requirements
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
