Job Summary

The RN-Patient Flow Operations Administrator operates within Prisma Health Patient Flow Operations, encompassing Central Bed Management and the Transfer Center. Central Bed Management oversees the placement of inpatient and observation patients by reviewing admission/discharge/transfer (ADT) orders and bed requests in coordination with clinician assessments. The Transfer Center facilitates direct admissions and patient transfers from both Prisma Health and external facilities and physician practices. Together, the Prisma Health Patient Flow Operations teams coordinate patient admissions, transfers, and placements in alignment with each facility or unit's capacity and capabilities. This role leads and supports initiatives to maximize throughput and capacity in the acute care setting. Monitors timely transitions of care across the continuum to include Market Emergency Departments, Inpatient units, and Procedural Areas.

Essential Functions

  • Serves as an "air traffic controller" for patient flow operations within assigned market. Ensures timely and efficient movement of patients across the system.

  • Coordinates and triages transfer requests to optimize capacity and support patient-centered care.

  • Monitors and evaluates throughput metrics, in collaboration with campus leadership teams, to develop action items to address throughput opportunities and improve processes.

  • Utilizes critical thinking skills and nursing autonomy to prioritize patient needs, in conjunction with standardized processes for patient admissions, transfers, and discharges.

  • Collaborates with physicians, clinical providers, care management, nursing and other members of the care team to escalate and eliminate barriers impeding efficient patient flow within the assigned market.

  • Creates and executes plans to ensure smoothing of patient throughput, length of stay management, and other efforts to deliver continuous, predictive, and optimal bed capacity management within Prisma Health.

  • Maintains oversight of patient transfers, admissions, and discharges to ensure alignment with organizational goals and market throughput objectives. Supports efforts to improve patient throughput and ensure safe, timely care transitions for our patients across the Market.

  • Contributes to patient safety by reporting safety concerns, errors, and near misses and by attending daily safety huddle and reviewing occurrence reports.

  • Serves as the operational liaison between the Transfer Center, Providers, Emergency Departments, Inpatient Units, Administrators on Duty (AOD's), EMS Transports, Environmental Services, and other members of the interdisciplinary care team to improve care transitions and resolve market bottlenecks.

  • Acts as a resource for other staff members by maintaining a strong clinical knowledge and understanding of individual department capabilities.

  • Delegates tasks as appropriate to other members of the team and ensures completion of assigned tasks.

  • Utilizes data and real-time situational awareness to make informed decisions, anticipate challenges and ensures timely elevation of urgent requests/needs to Transfer Center Leadership.

  • Monitors system-wide capacity and collaborates with operational leaders to implement strategies that improve patient throughput and resource utilization.

  • Evaluates own nursing practice in relation to professional practice standard guidelines, state statutes, policies, procedures, rules/regulations and current specific knowledge. Takes personal accountability for maintaining competency through education and professional activities. Maintains professional licensure and completes required mandatory education in a timely manner. Reflects on identified areas of strength that offer opportunities for improvement.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Associate degree in Nursing. Bachelor's degree in nursing preferred.

  • Experience - Three (3) years of experience as a registered nurse.
