Job description

Description FBS Surrogate Provider Requirements
  • High School diploma or GED required.
  • Must be at least 18 years old.
  • Must be able to work a flexible, part-time schedule.
  • One (1) year's volunteer or paid work experience in a human services field (i.e. education, mental health, nursing home or medical environment) .
  • Must have a current, valid driver's license and good driving record.
  • Ability to communicate effectively with families and maintain positive working relationships with individuals receiving services and care team. Must be able to work with diverse groups of people with varying cultural and educational backgrounds.
  • Must be able to complete New Hire Training within 60 days of hire date.
