Role Overview

As a Hospice Chaplain / Spiritual Counselor at VitalCaring, you are more than a spiritual support resource - you are a trusted guide, compassionate listener, and calming presence for patients and families navigating grief, transition, and end-of-life experiences.

You'll provide individualized spiritual care aligned with each patient's beliefs, values, and preferences while supporting emotional well-being and helping families process difficult moments with dignity and compassion.

Through every visit, conversation, and interaction, you help bring peace, reassurance, and hope.

What You'll Do

Provide Compassionate Spiritual Care

Assess spiritual and emotional needs of patients, caregivers, and families

Develop and support individualized spiritual care plans

Provide spiritual counseling, emotional support, grief support, and compassionate presence

Support patients and families through difficult conversations, transitions, and end-of-life experiences

Serve as a Collaborative Care Partner

Participate in interdisciplinary team (IDT) meetings and care planning

Collaborate closely with nurses, social workers, physicians, aides, and community clergy

Help ensure holistic, patient-centered care aligned with patient goals and wishes

Coordinate visits from local clergy or faith leaders when appropriate

Support Patients & Families

Build meaningful, trusting relationships with patients and loved ones

Provide comfort and guidance while respecting diverse faiths, beliefs, cultures, and lifestyles

Help families navigate grief, anticipatory loss, and emotional stress with empathy and sensitivity

Serve as a calming and supportive presence during deeply personal moments

Drive Quality & Documentation

Maintain accurate and timely clinical documentation

Participate in quality and compliance initiatives

Support hospice philosophy and agency standards of care

Maintain ongoing professional development and continuing education

What You Bring

Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree required)

Knowledge of and commitment to hospice philosophy of care

Ability to work effectively within an interdisciplinary team environment

Comfort serving within an interfaith setting

Ability to support individuals from diverse cultures, lifestyles, beliefs, and backgrounds

Excellent communication, empathy, and active listening skills

Experience working with death, dying, grief, and family/caregiver support

Valid driver's license, reliable transportation, and current automobile insurance for field-based responsibilities

What Sets You Apart

Hospice, palliative care, or healthcare spiritual care experience

Clinical Pastoral Education (CPE) experience

Strong grief counseling and emotional support skills

Ability to quickly build trust and meaningful relationships

Established relationships with community clergy and faith-based organizations

Experience documenting within an EMR platform
