Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. Spiritual Coordinator: South Hills
At Concordia Visiting Nurses, we believe care goes beyond the physical. We are looking for a compassionate and faith-driven PART-TIME Spiritual Care Coordinator to provide emotional and spiritual support to patients and families during some of life's most meaningful and vulnerable moments.

Who You Are as a Spiritual Care Coordinator
You are someone who leads with compassion, empathy, and presence. You understand the importance of meeting patients and families where they are-emotionally, spiritually, and personally. You are comfortable supporting individuals from a variety of faith traditions, beliefs, and backgrounds, and you know how to offer comfort during difficult and often sacred moments.
You are a strong communicator, a calm presence, and someone who truly values holistic patient care.
We are seeking someone who has:
  • Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting
  • A compassionate and patient-centered approach
  • The ability to provide support to patients and families from diverse backgrounds and belief systems
  • Strong communication and documentation skills
  • The ability to work independently while collaborating with an interdisciplinary team
  • A valid driver's license and reliable transportation for travel within the service area

What You'll Do
  • Provide spiritual support and counseling to patients, families, and caregivers
  • Assess spiritual needs and help incorporate them into the patient's overall plan of care
  • Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care
  • Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team
  • Participate in care planning and team meetings
