Overview
Our Visiting Registered Nurse (VNA) Certified division provides short- and long-term home care services for patients covered by Medicare, Medicaid, Private Insurance, and Private Pay. Services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients' homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs.
Pay
$40-45 per hour/visit based on the pay point model

What You'll Do
  • Serve as the primary clinical partner for patients and families, delivering individualized, one-on-one nursing care in the home that promotes recovery, safety, and independence.
  • Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, and translate those findings into thoughtful, patient-centered care plans.
  • Provide skilled nursing interventions-including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management-tailored to each patient's goals and condition.
  • Educate and empower patients and caregivers through clear instruction, coaching, and support, helping them confidently manage symptoms, medications, and daily care at home.
  • Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary care team to adjust plans and prevent avoidable hospitalizations.
  • Coordinate care across providers and services, ensuring continuity, clarity, and high-quality outcomes throughout the patient's home health journey.
  • Complete timely, accurate documentation that reflects the full clinical picture and supports quality, compliance, and measurable outcomes.

Qualifications
  • An active RN License in the state of application
  • Current CPR certification (with hands-on component) or able to obtain one
  • Current TB test
  • Must have drivers license, reliable transportation, and auto insurance

Preferred
  • Medicare Skilled Nursing experience
  • Basic understanding of OASIS
  • 1-year RN experience in a health care setting
