Job Summary
The Registered Nurse (Home Health) with OASIS experience provides skilled nursing care to patients in their homes while ensuring compliance with federal and state regulations. This role involves conducting comprehensive patient assessments, developing and implementing individualized care plans, and documenting OASIS data accurately to support quality patient outcomes and reimbursement requirements.
The RN collaborates closely with patients, families, and the interdisciplinary care team-including therapists, social workers, and physicians-to coordinate care, monitor progress, and provide education and support. This position requires strong clinical expertise, attention to detail, and the ability to work independently in a home care environment.
Qualifications
  • Minimal of two years of previous Home Health experience
  • Must be available on Mondays and Fridays
  • A current license as a Registered Nurse in Connecticut
  • OASIS experience required
  • Have reliable transportation and be willing to commute to provide the best in-home patient care
What You'll Do
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions; reporting changes to Nurse Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments as necessary
  • Collaborate with other professionals
  • Manage home health aides or CNAs
  • Weekend Rotation will be required. Typically, this is one weekend per month or less.

Compensation
Pay: $65.00 - $75.00 per hour
