Become the Pharmacist you always wanted to be
Computer proficiency/Clinical Systems:
  • Obtains census and confirmed admissions from Virtual Admission Board (VAB) from each of the facilities.
  • Uses Microsoft Word and Excel to prepare basic documents and analysis.
  • Uses remote connectivity to access clinical systems for all hospitals serviced.
  • Understands and can demonstrate the use of Powerchart and PharmNet to enter, verify and manage medication orders.
  • Uses appropriate online resources to research and respond to drug information questions.
  • Reviews system alerts for drug interactions, allergies, order & therapy duplications, over-dose range, and renal dosing and follows appropriate process to resolve issues.
  • Documenting Activities:
    • Documents clinical intervention, which may include, but not limited to, renal dosing, vancomycin dosing, and explanation of medications issues and determine if follow-up is necessary.
    • Complies with individual hospital reporting requirements to communicate resolved and ongoing issues with hospital-based pharmacists.
  • Formulary Compliance:
    • Demonstrates familiarity with the drugs that are formulary and non-formulary at each hospital serviced and provides the names of cost-effective formulary medications when asked for a recommendation by a physician.
    • Reviews TNF orders and converts to valid product order when possible.
  • Other Clinical or Remote Pharmacy Activities:
    • Manages all generated protected health information in a manner consistent with the privacy rule as outlined in HIPPAA.
    • Calculates appropriate dose and interval for drugs requiring pharmacokinetic monitoring.
    • Calculates and/or reviews creatinine clearance and recommends appropriate dosing based on renal function.
    • Applies individual hospital policies, protocols/procedures and process to each specific hospital's environment, during documentation and verification of medications.
    • Evaluates reported lab values in determining appropriateness of medication therapy.
    • Determines if the medication list provided with the patient is appropriate to use according to each hospital's protocol, and accurately document transfer/discharge, home and ER medication lists into Cerner in a timely manner. Identifies problem issues and alerts verification pharmacist.
    • Reviews controlled substance orders for appropriate dosing, duplication, and duration of therapy.

Qualifications
  • Licensed in good standing by the State Pharmacy Board. For the State in which the hospital is located.
  • Successful completion of a degree from an accredited pharmacy program or one that is determined to be substantially equivalent to a U.S. accredited program by an appropriate certifying agency.
  • Minimum 1 year of supervisory experience.
