Job description

Position Overview

We are seeking a compassionate and licensed Physician Assistant (PA) to join our team on a part-time,

remote basis.

In this role, you will conduct brief monthly telehealth follow-up visits with patients using Continuous

Glucose Monitoring (CGM) devices. Your primary responsibility is to review each patient's CGM reports,

discuss glucose trends, provide education and personalized recommendations, and determine any

necessary next steps to optimize diabetes management.

This is an excellent opportunity for a provider seeking a flexible schedule with minimal administrative

responsibilities while making a meaningful impact on patient care.

Responsibilities

  • Conduct monthly telehealth follow-up appointments with patients using CGM devices.
  • Review, interpret, and discuss CGM reports and glucose trends with patients.
  • Educate patients on their glucose patterns and provide lifestyle, nutrition, and diabetes management recommendations.
  • Identify patients who require additional evaluation, treatment, or follow-up.
  • Accurately document patient encounters in the electronic medical record.
  • Collaborate with the Care4Life clinical team to ensure continuity of care.

Qualifications:

  • Active New York State Physician Assistant license in good standing with no current disciplinary actions (required).
  • Experience in primary care, endocrinology, internal medicine, or diabetes management is preferred.
  • Experience working with Continuous Glucose Monitoring (CGM) programs is preferred but not required.
  • Comfortable reviewing and interpreting CGM reports and glucose trends.
  • Excellent communication and patient education skills.
  • Experience providing telehealth services is a plus.
  • Reliable internet connection and a private, professional workspace for virtual patient visits.
