About the Role:

The Community Outreach Team is a dedicated group responsible for promoting Foodsmart's foodcare program in partnership with Santa Clara Family Health Plan (SCFHP). This part-time contractual position plays a pivotal role in increasing enrollment in the Foodsmart program by identifying community partners and events, and creating a strategy to increase enrollment through onsite activations and referrals.

We are seeking a candidate who possesses strong interpersonal skills, a deep knowledge of the Santa Clara County community, and a passionate belief in Foodsmart's commitment to food as medicine. The ideal candidate will have experience navigating the diverse communities of Silicon Valley — including large Latinx, Vietnamese, Filipino, and South Asian populations — and will bring an on-the-ground presence to connect SCFHP members to Foodsmart's nutrition support services.

Please note that this is a part-time position with up to 25 working hours each week. The individual in the role must be willing and able to travel to various locations across Santa Clara County and surrounding Bay area.
 
You will:
  • Identify active Community-Based Organizations (CBOs), Faith-Based Organizations (FBOs), Federally Qualified Health Centers (FQHCs), and providers that serve the Santa Clara Family Health Plan member population across Santa Clara County.
  • Identify and evaluate local community events, health fairs, and clinic-based opportunities for onsite Foodsmart presence in Santa Clara County.
  • Attend enrollment events to raise awareness of Foodsmart's foodcare program and SCFHP benefits.
  • Cultivate relationships with identified organizations to build awareness of Foodsmart.
  • Define scope of partnership with each organization, including onsite events and referral pathways.
  • Work with the Foodsmart marketing team to create materials for events with local organizations — including flyers, recipe cards, and appointment confirmation materials in various languages.
  • Provide education to diverse and underserved populations about healthy eating, chronic disease prevention and management, and SNAP/CalFresh benefits.
  • Screen event attendees for Foodsmart eligibility and schedule nutrition coaching appointments with registered dietitians.
  • Motivate and encourage eligible members to complete nutrition programming and screening assessments (including the Hunger Vital Sign) for which they qualify.
  • Maintain a comprehensive activity log tracking events, referrals generated, appointments scheduled, and enrollment outcomes to share with the internal CHOC team.
  • Attend bi-weekly team meetings and calls with the internal Foodsmart team and SCFHP client team as needed.
  • Support the CHC (Community Health Center) referral activation strategy, including outreach to FQHCs and CBOs across Santa Clara County.
You are:
  • Based within Santa Clara County and able to travel throughout the county and surrounding Bay Area as needed.
  • Bilingual in English and Spanish (required).
  • Proactive and community-rooted, acting as an advocate for the populations you serve.
  • Comfortable working in situations that involve confidential and sensitive member information.
  • Skilled at building effective working relationships with colleagues, senior staff, clinical partners, and community members.
  • Proficient in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint) and comfortable learning new platforms (e.g., ReferralMD, Salesforce, or similar CRM tools).
  • Willing and able to travel to community events, health centers, and FQHC sites throughout Santa Clara County.
You have:
  • Bilingual fluency in English and Spanish (required).
  • The ability to travel to work sites throughout Santa Clara County and surrounding areas (required).
  • Strong organizational skills and the capacity to maintain detailed tracking of event enrollment, referral activity, and partnership outcomes.
  • Experience working with Medi-Cal populations, managed care plans, or community health programs in the Bay Area preferred.
  • Familiarity with the Santa Clara County health ecosystem — including Valley Medical Center, Santa Clara County Public Health, and the network of FQHCs and CBOs in the region — is highly valued.
  • The flexibility to handle additional duties as assigned and adapt to evolving program priorities.
