Position Overview

The Certified Nursing Assistant (CNA) provides direct, hands-on care and support to residents in accordance with individualized care plans. CNAs work under the direction of a Registered Nurse (RN) or licensed medical professional and play a critical role in supporting residents’ daily health, comfort, dignity, and independence.

This role focuses on assisting with activities of daily living (ADLs), monitoring residents’ conditions, documenting care, and communicating changes to the nursing and program leadership teams.

FLSA Status: Part-Time, Non-Exempt (Hourly)


Reports To: Senior Director of Programs (with day-to-day clinical direction from the RN)


Essential Job Functions

  • Assist residents with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, mobility, and feeding
  • Support residents in maintaining personal hygiene, comfort, and safety
  • Observe and report changes in residents’ physical or behavioral condition to the RN or supervisor
  • Assist with vital signs, basic health monitoring, and non-invasive care as directed
  • Support residents with mobility, transfers, and positioning using proper body mechanics
  • Provide support during meals, including preparation and feeding assistance when required
  • Accurately document care and observations in Therap or other required systems
  • Follow Individual Support Plans (ISPs) and health care plans as directed by nursing staff
  • Assist with maintaining a clean, safe, and organized living environment
  • Communicate respectfully and clearly with residents, families, and team members
  • Support residents’ community involvement and daily routines as appropriate
  • Adhere to all OAR regulations, agency policies, and confidentiality standards
  • Respond appropriately in emergency situations and follow established protocols
  • Maintain professional boundaries and a compassionate, person-centered approach at all times

Minimum Requirements

  • Active CNA certification in the state of Oregon
  • Current CPR certification (or ability to obtain upon hire)
  • Knowledge of basic caregiving and resident support practices
  • Ability to follow care plans, instructions, and agency policies
  • Strong interpersonal and communication skills
  • Ability to remain calm and responsive in high-stress or emergency situations
  • Basic computer skills; Therap experience preferred but not required
  • Ability to work collaboratively as part of a multidisciplinary care team
  • Must pass Oregon criminal background screening
  • Must be able to remain awake, alert, and attentive while on shift

Physical Requirements

  • Ability to lift up to 45 lbs
  • Ability to bend, squat, stand, and assist with transfers
  • Ability to walk up and down stairs
  • Ability to perform job duties with or without reasonable accommodation
