Community Wellness RN Position Summary

The Registered Nurse in Community Wellness works in close collaboration with the Nurse Practitioner to provide screenings, wellness exams, and urgent care to community members at different locations in the tri-county area. The RN is responsible for obtaining and documenting pertinent information including vital signs, demographics, chief complaints, social concerns, medical histories, and other information necessary to provide ongoing collaborative care of the patient in a multidisciplinary team approach. The RN is also responsible for referring patients to appropriate community services and assisting the patients in following through with the referrals. The RN will be expected to follow up with the patients seen on the van to ensure they have the resources they need to be successful. This may be achieved through follow up phone calls or visits to the home or other locations.

Data collection and reporting are other important functions of the RN on the Wagner Van as well as the keeping of Medical Records. The ideal candidate will have an outgoing, warm personality that ensures levels of comfort and trust amongst the clients and a spirit of cooperation with community partners. The ideal candidate will also have excellent assessment skills, phlebotomy skills, and interpersonal skills and have a good working knowledge of chronic disease.

Community Wellness RN Position Requirements

  • Licensed and currently registered to practice as a Registered Nurse. Must hold a license in the state of permanent residency. 
  • American Heart Association Basic Life Support (BLS)
  • Experience in community health or patient education preferred
  • Excellent assessment skills, phlebotomy skills, and interpersonal skills are required.
  • A good working knowledge of the pathophysiology of chronic disease and treatment options are preferred

Community Wellness RN Work Schedule

  • The van is staffed day shift, Monday through Friday.
  • Occasional weekend hours may be required.
  • Expected to respond in the event of a disaster.

Community Wellness Registered Nurse (RN) Benefits

  • At TidalHealth, full-time employees working at least 72 hours per pay period, part-time employees working at least 36 hours per pay period and part-time employees working at least 30 hours or more on weekends only are eligible for benefits.
