JOB SUMMARY:
Acadian Health has an exciting opportunity for Registered Nurses to join our team. The Registered Nurses deliver hospital-level acute care in a patient's home as part of an innovative care model that combines technology, clinical expertise, and interdisciplinary collaboration. The RN will manage both acute episodes and ongoing clinical needs in a non-traditional setting while functioning as an essential link between patients, virtual care teams, and the broader- healthcare system.
DUTIES AND RESPONSIBILITIES:
  • In-Home Acute Clinical Care: Provide high-acuity care in the patient's home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery-offering an alternative to inpatient hospitalization.
  • Clinical & Environmental Assessments: Conduct thorough physical exams and assess the home environment for safety and factors that may impact recovery or the plan of care. Identify and escalate any deviations that could compromise patient outcomes.
  • Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team. Maintain real-time communication with the remote command center nurse and physician to report findings, collaborate on care decisions, receive treatment orders, and ensure all aspects of the care plan are executed safely and effectively.
  • Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts in the home environment, providing necessary interventions under established protocols and medical control guidance.
  • Patient & Caregiver Education: Deliver personalized education regarding disease management, medication regimens, treatment expectations, and when to seek help. Use both verbal coaching and written materials to reinforce understanding.
  • Documentation & Compliance: Document all assessments, interventions, communications, and outcomes in the electronic health record (EHR) system in real time. Ensure care is delivered according to program protocols and clinical standards.
  • Interdisciplinary Collaboration: Collaborate with physicians, advanced practice providers, case managers, social services, and community partners to ensure continuity of care. Act as a liaison between the patient and healthcare system to eliminate gaps in care or missed services.
  • Program Support & Leadership (as assigned): Participate in research initiatives, support program development, and mentor new staff as directed by leadership. Represent the program in community outreach or quality improvement activities when needed.

REQUIRED QUALIFICATIONS:
  • Active RN license (in good standing)
  • Current BLS and ACLS (American Heart Association)
  • Minimum 3 years of experience in Med/Surg or Telemetry preferred
  • Strong decision-making skills in dynamic and autonomous environments
  • Excellent verbal and written communication skills
  • Proficiency with electronic health records and digital tools
  • Reliable transportation and valid driver's license
  • Comfortable providing clinical care in non-traditional home settings

PREFERRED QUALIFICATIONS:
  • Bachelor of Science in Nursing (BSN)
  • Emergency department, critical care, EMS, or mobile integrated healthcare experience
  • Previous experience as an educator, preceptor, or trainer
  • Community Paramedicine certification or willingness to obtain
  • Familiarity with virtual care platforms or command center models
