Position: Full-Time Primary Care Physician (Geriatrics) - Multiple San Diego Locations - Senior Living Facilities

(Part Time may be an option)

Job Type: Full-Time Schedule: Monday to Friday (No Weekends) OR Part-Time (No Weekends)

Location: San Diego Area

About Us: Aligned Geriatrics is a rapidly growing mobile medical practice that specializes in Geriatric Medicine. Our mission is to provide high-quality and compassionate care for seniors in the comfort of their homes or senior living communities. As a primary care provider with Aligned Geriatrics you will play a crucial role in improving the lives of older adults by delivering personalized healthcare services.

Physician Responsibilities:

  • Conduct regular rounding at assisted living and memory care facilities, managing both acute and chronic conditions.
  • Run multiple in-house clinics at senior living communities with set hours each week seeing independent, assisted living and memory care patients. (Patients are pre-scheduled).
  • Complete history and physicals on new admissions and oversee care transitions.
  • Respond to urgent medical issues during facility hours.

  • Collaborate with nursing, therapy, and administrative staff to ensure coordinated care.

  • Communicate with families and responsible parties regarding treatment plans and changes.

  • Document encounters and medical decision-making clearly and timely in the EMR.

  • Participate in quality assurance and performance improvement activities.

  • Follow all federal, state, and facility-specific regulations and protocols.

Qualifications:

  • Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
  • Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred)
  • Active and unrestricted California medical license
  • Experience in Geriatric Medicine and/or primary care is a plus
  • Strong communication and interpersonal skills
  • Experience with Epic (EHR) is a plus
  • Experience with Microsoft Office is a plus
  • Strong time management and organizational skills
  • Comfortable working independently and in team based environments
  • Compassionate bedside manner with a commitment to providing patient centered care.
  • Experience in skilled nursing or post-acute care preferred.
  • Strong communication and clinical decision-making skills.

  • Ability to work independently while collaborating effectively with facility staff.
