Linwood Meadows Care Center
Pay Range: $28-$31 per hour

Position Summary

Linwood Meadows Care Center is seeking a dedicated Part-Time Licensed Vocational Nurse (LVN) to provide direct nursing care to residents and supervise the daily activities of Certified Nursing Assistants (CNAs).

The LVN ensures care is delivered in accordance with federal, state, and local regulations, as well as facility policies and direction from the Director of Nursing or Nurse Supervisor, to maintain the highest level of quality resident care.

Essential Duties and ResponsibilitiesNursing Care & Clinical Responsibilities
  • Provide direct nursing care in accordance with physician orders and resident care plans.

  • Administer medications and treatments as prescribed.

  • Monitor residents' conditions and report changes to physicians and family members as appropriate.

  • Perform skilled nursing procedures including wound care, catheterization, tube feedings, suctioning, specimen collection, and other treatments as required.

  • Take and record vital signs and document resident status accurately and timely.

  • Participate in admissions, transfers, and discharges.

  • Assist with discharge planning and care coordination.

  • Ensure resident care is delivered in accordance with individualized care plans and facility standards.

Supervision & Leadership
  • Direct and supervise CNAs during assigned shifts.

  • Conduct shift rounds to ensure nursing care standards are maintained.

  • Provide guidance, support, and leadership to assigned nursing staff.

  • Communicate staffing needs or concerns to the Nurse Supervisor.

  • Promote a calm, respectful, and positive environment for residents and staff.

Documentation & Compliance
  • Accurately transcribe and implement physician orders.

  • Complete required documentation including nurses' notes, incident reports, and medication records.

  • Ensure all entries in resident medical records are timely, complete, signed, and dated.

  • Maintain confidentiality of resident information in compliance with HIPAA.

  • Participate in quality assurance activities and facility surveys.

  • Follow infection control, safety, and regulatory guidelines at all times.

Supervisory Responsibilities

As an LVN, you are delegated the authority, responsibility, and accountability necessary to supervise nursing assistants and ensure quality care during your assigned shift.

QualificationsEducation and Experience
  • Graduate of an approved LVN program.

  • Current, active, and unencumbered LVN license in the State of California.

  • Experience in a skilled nursing facility preferred.
