Position Summary

As a Part-Time LPN at Luna Del Valle Hospice, you will deliver direct patient care in the home setting, under the supervision of a Registered Nurse (RN). This role is essential to ensuring high-quality, compassionate care that respects patient dignity and supports families during a vulnerable time.
Responsibilities

  • Administer medications and treatments per Hospice policies and clinical standards
  • Perform baseline nursing skills with clinical competency and in accordance with state practice acts
  • Monitor patient condition and promptly report any changes, needs, or concerns to the supervising RN
  • Assist in developing and updating patient-centered Plans of Care, including measurable goals and interventions
  • Maintain accurate and timely documentation of nursing interventions, observations, and education provided
  • Provide emotional support to patients and families, always upholding patient rights and dignity
  • Communicate effectively with physicians, interdisciplinary team members, patients, and families
  • Participate in team conferences and contribute to collaborative care planning
  • Promote teamwork by supporting and assisting fellow staff members when needed
What You Bring

  • Graduate of an accredited Licensed Vocational/Practical Nursing Program
  • Current and valid LPN license in the State of New Mexico
  • CPR certification and a valid drivers license with auto insurance

  • At least 1 year of nursing experience (Hospice experience preferred)
  • Strong organizational, time management, and communication skills
  • A cooperative, compassionate approach to patient care
  • Ability to handle emotional demands with grace and professionalism
  • Basic math skills for medication and treatment administration
