Job description

PeaceHealth is seeking a RN Hospice House- Ray Hickey Hospice House for a Part Time, 0.70 FTE, Evening position. The salary range for this job opening at PeaceHealth is $50.14 - $78.34. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

This position is responsible for providing direct skilled nursing care and coordination/management of patients admitted to hospice house inpatient services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation.
Essential Functions
  • Perform initial evaluation of patient and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
  • Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the hospice plan of treatment.
  • Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely hospice outcomes.
  • Develops and clinically manages the Home Health Aide's care plan according to the patient's evolving personal are needs.
  • Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
  • Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
  • Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.
  • Assist with the plan and implementation of family Bereavement services for the following patient death.
  • Performs other duties as assigned.

Qualifications
Education
  • Accredited School of Nursing Required: Nursing and
  • Bachelor's Degree Preferred: Bachelor of Science in Nursing

Experience
  • Minimum of 1 year Required: home health or hospice/end of life nursing experience and
  • Preferred: Hospice experience and
  • Required: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs

Credentials
  • Required: Upon Hire Registered Nurse in state of practice and
  • Required: Upon Hire Basic Life Support and
  • Required: Upon Hire Food Handlers Card - Washington

Skills
  • Basic computer skills including MS Office applications (Required)
  • Well organized, flexible and able to function independently (Required)
  • Cooperative working skills, leadership skills, and interpersonal skills and ability to work effectively with a wide variety of patients, families and staff (Required)

Working Conditions
Lifting
  • Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
  • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
  • Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
  • Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
  • Ability to move around area with frequent sitting.
  • Bending/ stooping/ squatting/ reaching/ kneeling frequently.
  • Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

Environmental Conditions
  • Exposure to biohazard, body fluids and airborne particles.
  • Must be able to complete tasks in a noisy environment.

Mental/Visual
  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
