Job description

Join our compassionate team as an LPN/RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. At Auburn Crest Hospice our mission is to create a culture of excellence that honors life by empowering employees, patients and communities to live every moment!
This position is part-time weekends, particularly Sunday. Open for PRN throughout the week as well.
What You'll Do:
  • Manage patients, ensuring they receive appropriate care.
  • Complete assessments, create care plans, and document care.
  • Collaborate with doctors, social workers, and other team members.
  • Provide support and education to families.
  • Enjoy flexible scheduling and work-life balance (avoid burnout!).

We're Looking For:
  • Idaho RN/LPN License.
  • 2+ years of experience (pref. hospice).
  • Strong communication and teamwork skills.
  • Valid driver's license & reliable transportation.

Certifications, Licenses and Registrations
  • Registered Nurse licensed in the state of employment.
  • Minimum of one (1) year nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years is preferred.
  • Certification in Hospice and Palliative Nursing a plus but not required.
  • Current CPR certification.
  • Must have a valid driver's license and auto liability insurance.
