About the Role:
HOMECARE RESCUE LLC is looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing Home Health team serving Pasco County. Candidate will initiall work on a PRN basis.  This is a rewarding opportunity to make a real difference in patients' lives by delivering high-quality, one-on-one care right in the comfort of their homes. Responsibilities:
  • Provide skilled nursing care to home health patients throughout Pasco County in accordance with physician-approved care plans
  • Perform clinical assessments, wound care, medication administration, and IV therapy as ordered
  • Monitor and document patient progress, vital signs, and changes in condition
  • Educate patients and family members on disease management, medications, and home care techniques
  • Collaborate with RNs, therapists, and the interdisciplinary care team to ensure continuity of care
  • Maintain accurate and timely clinical documentation in compliance with state and federal home health regulations
  • Communicate patient status updates and concerns to supervising RN or physician promptly
Requirements:
  • Active Florida LPN license in good standing
  • Familiarity with WellSk
  • Previous home health, long-term care, or clinical nursing experience preferred
  • Strong knowledge of wound care, medication management, and patient assessment
  • Reliable transportation and valid driver's license for travel throughout Pasco County
  • Excellent communication, time management, and organizational skills
  • Ability to work independently with minimal supervision in a home care setting
  • CPR/BLS certification required
