Position Overview

Key Responsibilities and Qualifications

The Registered Nurse will provide coordinated nursing care to patients of various age groups in their homes. This role requires accountability and collaboration with an interdisciplinary team to set and achieve patient goals while ensuring high-quality care. The nurse will operate under the supervision of the Clinical Manager and in accordance with physician orders.

Responsibilities include:

  • Evaluating home care patients to identify their physical, psychosocial, and environmental needs, as documented in clinical records, case conferences, team reports, call logs, and on-site assessments.
  • Completing OASIS assessments and visit documentation per agency policy, ensuring clinical notes reflect ongoing communication and coordination with physicians, team members, and patients/families/caregivers.
  • Reporting significant findings, issues, and changes to the Clinical Manager and physician, while documenting all communications and interventions.
  • Overseeing and providing clinical guidance to home health aides and LPNs/LVNs to guarantee quality and continuity of care.
  • Participating in on-call rotations and emergency calls as per agency policy.
Required Qualifications:
  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • Valid Driver's License.
  • Registered Nurse (RN) license.
  • Associate Degree in Nursing.
