Job Title: Home Health Physical Therapist - Per Diem
Hourly Rate Range: $60 - $70 per hour
Coverage Area: San Joaquin County, California

Responsibilities:

  • Current CA Physical Therapist license
  • Graduate of an accredited PT program
  • CPR certification
  • Valid driver’s license, insured and reliable vehicle
  • Minimum 2 years PT experience (home health preferred but not required)
  • Strong communication and interpersonal skills
  • Knowledge of PDGM Medicare model a plus
