Job Summary
Home Services Unlimited is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide in-home care throughout the Indianapolis area. This position offers autonomy, flexibility, and the opportunity to build lasting relationships with patients while managing your own caseload.

• Perform comprehensive in-home patient assessments (including OASIS)
• Develop, implement, and evaluate individualized care plans
