Prior home health or hospice experience is preferred but not required — we are happy to train the right candidate. This role is well-suited for an RN who values autonomy, meaningful patient relationships, and providing care with dignity and heart. As a CHAP-accredited organization, we’re committed to high standards, ethical care, and supporting our clinicians every step of the way.


Responsibilities
  • Evaluate the effectiveness of nursing services provided to the patient and family on an ongoing basis
  • Perform admissions, transfers, recertifications, resumptions of care, and discharge OASIS assessments for home health patients
  • Prepare and present patient records to the Clinical Record Review Committee, as indicated
  • Consult with the attending physician regarding alterations to patient care plans; coordinate with the appropriate supervisor and implement changes as needed
  • Make regular visits to patients to assess condition and progress
  • Coordinate patient services and collaborate with interdisciplinary team members
  • Submit clinical notes no less frequently than weekly, including progress notes and other required clinical documentation outlining services rendered
  • Provide support and comfort in accordance with individual patient needs
  • Work collaboratively with caregivers to plan and deliver high-quality patient care
  • Hospice: Alleviate symptoms associated with the end stage of disease
  • Hospice: Willingness to take on-call weekends when needed
  • Submit a daily tally of patient care visits completed
  • Perform other duties as outlined in the KCHAP job description


Requirements and Skills
  • Proven experience as a Registered Nurse; hospice or home health experience preferred
  • Knowledge of nursing care processes and medication guidelines, including side effects
  • Commitment to safety precautions and high ethical standards
  • Sound judgment and strong attention to detail
  • Excellent communication skills and a compassionate, patient-centered approach
  • Current CPR certification
  • Ability to reliably report to assigned patient locations
  • Valid driver’s license and ability to travel to patients’ residences
  • Ability to access patients’ homes, which may not be routinely wheelchair accessible
  • Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and patient care
  • Physical activity requirements include walking, sitting, stooping, standing, and lifting/turning patients
  • Ability to lift up to 50 pounds
  • Ability to communicate effectively in English, both verbally and in writing


Qualifications
  • Graduate of an accredited School of Nursing (documentation required)
  • Current, unrestricted Registered Nurse license in the state of Kansas (documentation required)
  • Knowledge of Medicare and Medicaid guidelines
  • Willingness to submit to a criminal background check
  • Current CPR certification through an approved provider (in-person skills verification required)
