Job description

Signature Healthcare at Homeis seeking to add an Admissions / SOC Nurse to our team!

The Signature Team is looking for an RN to be added to our admissions team in either a part-time or PRN basis and open to all locations within our territories (Island, Skagit and Snohomish Counties). This position has flexible hours and the ability to work weekends.

Duties and Responsibilities

  • Completes an initial and comprehensive physical assessment and history of current and previous illness(es) with patients.
  • Establishes the initial individualized plan of care in collaboration with the certifying medical provider based on comprehensive assessment and patient goals.
  • Participating in revising the plan of care as necessary in collaboration with the certifying medical provider as patient status and needs change.
  • Develops a care plan that establishes interventions and goals. Includes the patient and the family/caregiver in the planning process.
  • Works autonomously to initiate appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the medical provider in the plan of care.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Documents patient services and status timely and accurately within professional practice standards and reimbursement requirements.
  • Communicates with the medical provider regarding the patient's needs and reports changes in the patient's condition; obtains/receives medical provider orders as required.

Knowledge, Skills and Experience

  • Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home health preferred.
  • Currently licensed as a Registered Nurse in Washington State.
  • Possesses and maintains CPR certification.
  • Must be licensed and able to practice in Washington State.
  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.

Preferred Qualifications:

  • Clinicians with experience in Home Health or post-acute care.
  • Familiarity with Homecare Homebase EMR.
