Job description

Job Type: Contracted, Part-Time

Schedule: Self-determined – you can choose to set your schedule for:

  • Weekdays only (Monday-Friday)

  • Weekends only (Saturday & Sunday) or no weekends

  • Day or evening shifts

Location: On the road; locations will vary (in-person at the home(s) of client(s) living in and around the Austin area)

Qualifications and skills include:

  • REQUIRED: Possess a current PT license to practice in the state of Minnesota

  • REQUIRED: Valid Driver's License and reliable transportation

  • Ability to work independently

Responsibilities Include:

  • Evaluate and assess the patient’s current physical abilities and limitations and develop individualized treatment plans based on the patient’s needs

  • Provide physical therapy treatments and therapeutic exercises to clients in their homes

  • Monitor and document patient’s progress

  • Educate patient and family on how to manage and care for the patient’s physical limitations

  • Communicate with other healthcare staff to coordinate care for the patient
