A deep and diverse breadth of skills and experience bolster Safe Life's quality of care. Our large team of clinicians encompasses more specialties and disciplines such as wound-care certification, lymphedema management, other therapy specialties, and medical social work. Our coordinated, multidisciplinary approach to your healthcare enables us to send a team of professionals delivering the optimal experience and training for each patient's condition and presentation. Currently covering 13 counties in Illinois: Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee & Ogle.

Position Overview

Provides Occupational Therapy services to patients as directed by Plan of Care in line with the philosophy, objectives, and policies of the agency. Supervises Occupational Therapy Assistants and maintains responsibility for the implementation of follow through of OT procedures.

Responsibilities

  • Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.

  • Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, as appropriate coordinating plan of care with the physician as needed.

  • Prepares clinical and progress notes and required summaries, as appropriate.

  • Revises plan of treatment and contacts the physician for change in plan of care.

  • Advises and consults with patient’s family and other health care staff.

  • Coordinates care with agency DON and other staff as appropriate.

  • Observes, records, and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.

  • Supervises the Occupational Therapy Assistant.

  • Instructs patient and/or family in the use of equipment safe patient management and home therapy programs as needed.

  • Communicates effectively with patients, care givers, and families as well as agency staff.

  • Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.

  • Assures that all tasks are performed in a timely manner.

  • Checks in on Occupational Therapy Assistants every 4-6 patient visits.

Qualification

  • Must be a licensed Occupational Therapist by the State of Illinois.

  • Must have at least one-year Home Health Care clinical experience.

  • Car with required proof of insurance coverage.

  • Maintains annual TB test, CPR certification and other required.

  • Able to read and interpret instructions related to patient care.

  • Able to effectively perform and prioritize multiple functions or task as needed.

  • Must be able to travel to prospective patient’s place of residence.
