Ecumen Home Care-Twin Cities is a Medicare-certified, state licensed home health agency serving the Minneapolis Metro and surrounding areas and is located in Coon Rapids, Minnesota. We are currently seeking a Home Health Aide to join our team providing services in the Maplewood area. Ecumen creates home for older adults wherever they choose to live. Job Description
The Home Health Aide is responsible for providing personal care and other designated health-related services designed to maintain the client's physical and emotional well-being according to the physician-ordered plan of care.
Specific responsibilities include:
  • Follows the client's plan of care to help each client reach and maintain his or her highest level of performance and independence in the home environment. These services include: bathing, grooming and assisting with activities of daily living.
  • Provides communication with client and/or family while in client's home as appropriate.
  • Follows schedule frequency as ordered by physician.
  • Proactively participates in care conferences with other health team members as needed.
  • Maintains privacy and confidentiality of client, client's records and client's living environment. Understands and follows HIPAA guidelines.
  • Promotes personal safety and a safe environment for clients and co-workers.
  • Other duties as assigned.

Qualifications
The following are the required minimum qualifications for the Home Health Aide position:
  • High School Diploma or GED.
  • One (1) year experience as a Home Health Aide, Medicare home health experience preferred.
  • Certified Nursing Assistant (CNA) license.
  • Active status in the MN Nursing Assistant Registry.
  • Valid driver's license, current auto insurance and reliable vehicle.
  • Ability to communicate effectively, including written and verbal skills.
  • Ability to work well both individually and as part of a team, through respect, collaboration and dedication; successfully interact with a diverse culture base.
  • Ability to prioritize and organize work effectively and efficiently.
  • Demonstrated positive attitude, high integrity and ethical awareness.
  • Proven to be dependable and responsible.
  • Compassionate and caring attitude.
