Job description

We Are Hiring

Location: Hoffman Estates, IL

Facility: Ascension Saint Alexius 

Schedule: Day shift | Part-Time 32 hours 

Salary range: $39.75 - $56.11 per hour

What You Will Do
  • Develop and Implement Diabetes Programs: Create and oversee diabetes education programs that align with ADA standards, focusing on prevention, management, and patient empowerment.
  • Collaborate with Healthcare Providers: Work with physicians, nurses, dietitians, and other team members to create and implement individualized care plans for diabetes patients.
  • Provide Education and Support: Educate patients and families on self-management strategies, including lifestyle modifications, medication, and self-monitoring techniques.
  • Monitor Outcomes and Improve Care: Track patient progress, assess program effectiveness, and adjust strategies to ensure optimal diabetes management and adherence to ADA guidelines.
What You Will Need

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or jobtransfer date required.
  • Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Educator specializing in Diabetes Management credentialed from the American Heart Association (AHA) preferred.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time ofhire.
Additional Preferences
  • Registered Nurse with CDCES (Certified Diabetes Care and Education Specialist) preferred
  • Outpatient RN experience strongly preferred
  • RN experience working in an endocrinology office, IP acute care (med-surg/cardiology), IP Charge Nurse, or IP/OP Nurse Educator
