Overview

Provides individual and group education to persons with Diabetes, their families, and appropriate support systems about diabetes self-management and related issues. Addresses educational needs of other health care professionals who do not specialize in diabetes, and the public. Performs basic foot and nail care to individuals with diabetes. Administers diabetes health management education to children, pregnant women, adults, and elderly individuals. 

Responsibilities
  • Coordinates, plans, implements, and evaluates the Outpatient Diabetes Management Education series.
  • Provides information to the public and / or other health care providers regarding diabetes detection, treatment and management as requested and / or available. Maintains necessary criteria for ADA accreditation for Diabetes Education Program and for CLEA certification for blood glucose testing.
  • Implements, evaluates and assists with revision of standards of care and clinical practice protocols for Diabetes Management.
  • Utilizes problem solving skills to assist in identifying emerging patient care problems in area of specialty.
  • Documents patient care to meet requirements for JCAHO and reimbursement.
  • Participates in monitoring and evaluation activities which assess and improve nursing practice and patient outcomes.
  • Provides formal and informal consultation for individuals with diabetes management problems in the outpatient setting, hospital, and community.
  • Designs a multidisciplinary plan of care that provides for optimal patient outcomes.
  • Collaborates with homecare and other specialties in the Beebe Medical Center network in implementation and evaluation of the plan of care.
  • Provides education to patients, the community and other health care providers as deemed necessary and / or appropriate. Serves as a resource to other Beebe Medical Center health care professionals within area of expertise.
  • Participates in new technology and product evaluation.
  • Participates actively in Beebe Medical Center committees and meetings as deemed appropriate by supervisor.
  • Maintains membership in and participates in professional organizations.
  • Performs other duties within the scope of practice as designated by supervisor.
Qualifications
  • Minimum of five (5) years of professional nursing experience.
  • Proven leadership skills with the ability to guide, mentor, and collaborate effectively with healthcare teams.
  • Excellent verbal and written communication skills, with the ability to interact professionally with patients, families, physicians, and staff.
  • Demonstrated emotional intelligence, professionalism, and resilience in fast-paced clinical environments.
  • Strong commitment to delivering high-quality, patient-centered care and achieving positive patient outcomes.
