Job Summary

Lutheran Haven is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our night shift team. We are currently hiring for part-time opportunities.

The LPN is expected to assist in maintaining a positive physical, social, and psychological environment in accordance with the residents' needs and the facility's philosophy. The LPN is responsible to the Resident Care Coordinator for the planning, directing, supervision, and delivery of direct resident care. Additionally, the LPN is responsible for maintaining accurate and complete medical records, utilizing safety measures in the provision of care, and preventing the spread of disease and infection by following established universal precautions.

Job Duties
  • Actively supports Lutheran Haven's Mission and Ministry.
  • Demonstrates a willingness to be an effective team member by fostering positive relationships with team members and residents.
  • Provides consistent leadership to mentor and motivate staff toward quality resident care.
  • Visits with each individual resident three (3) times per week (generally Tuesday, Wednesday, and Thursday) to address resident needs and maintain communication with the Resident Care Coordinator.
  • Supervises daily functions of the Assisted Living Facility (ALF), including employee supervision and resident care delivery.
  • Gives and receives shift report at the beginning and end of each shift.
  • Makes initial rounds at the beginning of the shift on all assigned residents to assess comfort and safety.
  • Communicates immediately with the supervisor or administrator on call regarding any issues requiring immediate attention.
  • Administers personal hygiene, preventive, therapeutic, and rehabilitative services to residents and assists in coordinating medical care with other disciplines.
  • Admits new residents by evaluating their needs and reviewing physician orders.
  • Orders prescribed medications, supplies, and equipment in accordance with established policies; assists residents with self-administration of medications; performs approved nursing procedures and treatments; and maintains medication security, including narcotics, in accordance with facility policy and regulations.
  • Requisitions and arranges diagnostic and therapeutic services as ordered by the resident's physician.
  • Maintains accurate and complete documentation of resident care, condition, and treatment in compliance with HIPAA.
  • Enters accurate information from physician orders and medication records into the pharmacy Medication Observation Record upon resident discharge or transfer.
  • Performs other duties as assigned.
  Minimum Qualifications
  • Current Florida Licensed Practical Nurse (LPN) license required.
  • Minimum of one (1) year of verifiable clinical experience required; experience in geriatrics or rehabilitation is preferred.
  • Current CPR certification required.
