Registered Nurse Clinical Manager – Home Health

Exciting opportunity to be apart of a new BAYADA office opening in Fort Wayne, IN.

BAYADA Home Health Care is seeking an experienced and dynamic Clinical Manager in our Fort Wayne, IN Home Health office. This Medicare-certified intermittent skilled care office has proudly served the community for the past two years, providing short-term nursing, rehabilitative, therapeutic, and supportive home health services to adults and seniors through personalized one-hour in-home visits.

This leadership role is ideal for a motivated Registered Nurse who is passionate about clinical excellence, team development, and patient safety.

In this role, you will:

  • Lead, inspire and motivate clinical staff to provide outstanding patient care
  • Conduct clinical assessments and supervisory visits throughout Fort Wayne and the surrounding areas
  • Develop individuated care plans and oversee implementation of services
  • Supervise and support RNs, PT's, OT's SLP's and other field staff
  • Facilitate new hire orientation and onboarding
  • Conduct competency evaluations for both new and tenured staff
  • Review and monitor clinical documentation within Home Care Home Base
  • Ensure compliance with company policies, regulatory standards, and quality initiatives

Qualifications:

  • Minimum of 2 years experience as an RN
  • Prior Home Health and OASIS experience required
  • Acute care, medical-surgical, and/or sub-acute experience a plus
  • Strong clinical, communication, and organizational skills
  • Ability to build strong relationships with clients, families, and interdisciplinary teams
