Your future role at a glance

Location: Pensacola, FL

Facility: Ascension Sacred Heart Pensacola

Department: Case Management

Schedule: Days | Part-time | Weekend Option | 2 - 12 hour shifts

How you'll make an impact in this role
  • Interdisciplinary Care Coordination: Orchestrate the overall plan of care from admission to discharge, integrating patient and family preferences into a cohesive strategy across the entire continuum of care.
  • Specialized Psychosocial Assessment: Serve as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment.
  • Advocacy & Barrier Removal: Pro-actively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity.
  • Strategic Collaboration & Documentation: Maintain a continuous dialogue with the care transition team to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Clinical Counselor preferred.
  • Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
What additional requirements you'll need
  • Acute care case management experience preferred
  • MSW preferred
