Job Summary

Provides assistance to Nutrition Services /Case Managers in assessing the needs of clients, arranges and coordinates services/resources County, State and Federal programs.
Please note that this position is fully funded by grant funds. The expected duration of the grant funding is 3/31/2032. The position is funded for the duration of the grant and may be reduced or eliminated based on funding availability.
Essential Functions
  • Enters data into CIRTS network; answers the telephone; mails requested information; and makes copies.
  • Maintains client hard copy and electronic files.
  • Uses several different software systems on different platforms; uses word processing, spreadsheets, and CIRTS database.
  • Obtains holiday gifts and food for clients.
  • Collaborates with others when needed when establishing services for clients.
  • Performs other related duties as required.
  • Completes eligibility assessment of clients; assists Case Managers in developing a care plan to meet the service needs of the client; arranges and coordinates needed services for clients; and provides follow-up with clients as needed.
  • Maintains Federal, State, and County mandated client records; prepares reports; and writes letters and correspondence.

Minimum Qualifications
  • High school diploma or GED required.
  • One (1) year of related experience.
  • Fingerprinting required.

  • Salary offers above the minimum pay grade may be considered based on qualifications.
  • Essential Employees may be required to work during an undeclared or declared emergency. The County's Administrative Office will determine who will be required to work on an incident-by-incident basis.
