CATCH THE HOPE CASE WORKER POSITION SUMMARY

The Catch the Hope (CTH) worker meets with all identified patients (pregnant and post-partum) to provide education related to the CTH set curriculum. CTH workers are responsible for obtaining all necessary consents to release information. CTH workers will track all court dates. CTH workers may assist with finding appropriate housing/residential programs for patients upon release. All discharge planning related aftercare is coordinated by the CTH worker (medical, dental, vision, mental health, etc.). CTH workers provide Caregiving Affidavit and reviews Affidavit and all pertinent information. All patient contact will be documented, and a weekly census is to be sent out.

CATCH THE HOPE CASE WORKER MINIMUM REQUIREMENTS

  • A bachelor's degree in Mental Health or related field
  • Case Management experience
  • Applicants with experience in a correctional health care setting will be given preference.
  • Familiarity with medical/MH terminology/diagnosis should be required
  • Knowlege of accessing services in community
  • Licensing preferred, not required.
  • Baseline understanding the medical parole process preferred, not required.
