Job description

Stoughton Health has an excellent opportunity for an RN Case Manager to join our Medical- Surgical team on a Part-Time basis.
The RN Case Manager responsible for carrying out many duties that directly affect customer service, quality care, and hospital reimbursement. This is a multi-focal role involving review of benefits for planned admissions, concurrent review of charts for medical acuity, coordination of admissions to supportive care, participation in discharge planning within the multidisciplinary care team, assistance in assignment of DRG's, and contacts with hospital departments and outside agencies.

Position Requirements:

  • Required Education: Associate’s Degree in Nursing or experience commensurate to a Bachelor’s degree
  • Preferred Education: Bachelor's Degree. Advanced computer coursework.
  • Professional Experience: Requires five years acute care nursing experience. Prefer prior related work experience in Patient Review or Case Management.
  • Licensure: Currently licensed by the Board of Licensing and Regulation in the State of Wisconsin as a Registered Nurse in good standing
  • Computer Competency: Basic Computer Skills are expected for this position
  • Microsoft Office Suite Skills: Beginning Skills – Able to perform basic things such as opening a document, change font, save a document, write a letter.

Work Shift:

  • Part-Time, .5 FTE
  • Week 1 - 24 hours
  • Week 2 - 16 hours
  • Scheduled Days: Variable between Monday - Friday
  • Shift: 8:00 a.m. – 4:30 p.m.
  • No weekends
