Job Description
  • Ensure the individual care plans are implemented
  • Conduct a face-to-face evaluation of individuals as needed
  • Make weekly home visits
  • Follow-up with appointments, prescriptions, physician orders, discharge orders, etc.
  • Document service provided to each individual in a timely manner
  • Other related duties as assigned
Qualifications

Current RN License

2 years of experience in home health or related field

Experience working with special needs population

Knowledge of current home management and personal care methods

Additional Information

Basic Job Requirements Applicants must submit to a criminal background check before a an offer is extended (Employer Paid).

Other basic job requirements are as follows:

  • Must be able to pass a Criminal Background Check 
  • Tuberculosis Screening Test Results
  • Physical
  • CPR/First-Aid
  • Work 5-10 hours a week (paid direct deposit weekly)
  •  Recent 7 year Motor Vehicle Report copy if  hired 
  • Complete online orientation and annual training
