Physical Medicine & Rehabilitation of Long Island is expanding, and we are seeking a motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing team. Our practice is known for delivering outstanding, compassionate care to complex patients across Skilled Nursing Facilities throughout Suffolk County and the surrounding region. We specialize in pain management, wound care, amputee rehabilitation, neuro-rehabilitation, disability management, and comprehensive physiatric services.

Position Overview

The PA/NP will provide high-quality clinical care to patients in Skilled Nursing Facilities, focusing on:

  • Pain management evaluations

  • Wound assessment and treatment

  • Disability management and functional optimization

  • Neuro-rehabilitation, including stroke and amputee care

  • General Physiatry and complex medical rehabilitation consultations

  • Care coordination with facility staff, therapy teams, and families

You will become an integral member of the patient-care continuum, bringing compassion and expertise to a population that needs it most.


What Weโ€™re Looking For
  • Valid NYS PA or NP license

  • Experience in rehab, pain management, SNFs, wound care, or internal medicine is a plus (but not required)

  • Strong interest in Physiatry, disability medicine, and rehabilitation

  • Excellent communication, professionalism, and teamwork skills

  • Desire for long-term growth within the practice


Schedule & Structure
  • Per diem position preferred with flexible scheduling

  • Full-time opportunity available for the right candidate seeking expanded clinical responsibilities

  • Typical hours are weekdays during daytime; no nights or major holidays
